Privacy Notice: The following information is solicited under the authority of the Museum and Library Services Act, 20 U.S.C. 9101 et seq. Submission of your data in response to the below questions is voluntary. The data collected by this form will be used to help us identify and select individuals to participate in the Institute of Museum and Library Services' application review process. This information is covered under the E-Government Act of 2002 and/or the Privacy Act of 1974, 5 U.S.C. 552a. Excluding utilization for routine uses, IMLS will not disclose this information to anyone outside of the agency without your written consent or at your written request, unless it is legally required or specifically authorized by law.

We welcome your submission at any time. However, recruitment for the upcoming fiscal year closes in early December. Reviewer selection occurs from January through May.
