Under the Privacy Act, you may request access to, or correction or amendment of, records pertaining to you and contained in an IMLS System of Records.
To request for access, correction, or amendment of such records must be made in writing to the IMLS Office of the General Counsel (address below). You may also call the Office of the General Counsel during business hours at (202) 653-4787 to schedule an appointment to make a request in person.

Your written request should be marked as a “Privacy Act Request” on both the envelope and on the top of the request. The request should include:
- Your full name, email address (if applicable), mailing address, and daytime telephone number.
- A statement that you are seeking access or requesting correction/amendment of records under the Privacy Act, 5 USC §552a.
- A clear and specific description of the information you seek access to or want corrected/amended, and if applicable your reasons for requesting amendment or correction.
- The name of the IMLS system of records that you would like IMLS to search, as published in the Federal Register.
- Any other information which you believe would help IMLS determine whether information about you is included in the system of records, or information that would support your request to amend the pertinent record.
IMLS will respond within ten business days of the receipt of your request. To the extent IMLS has records responsive to your request, before providing access to and/or correcting or amending such records, IMLS will ask that you provide information adequately confirming your identity, and may charge fees, as required under the IMLS Regulations Implementing the Privacy Act.
